After installing this product, the temperature dropped to about 33 degrees C and under load, up to 38 degrees C. The fan is super quiet.
The only downside is that it does cover the WiFi chip and in some cases it might reduce the WiFi signal. But in my usage, I have the CAT5 cable plugged in so it really does not affect me.
I did some minimal WiFi/Bluetooth tests and the signal was still acceptable. Your mileage may vary.
Two notes of interest. The first is if you are going to use a camera or the display port, you might want to put those cables in place first because the heatsink fits very close to the connectors and you might have to use tools to install the cables or remove the heatsink temporarily.
The second note is that this heatsink DOES NOT cover the power or hdmi ports at all. It does not impede connectivity to these connections at all.
One recommendation for anyone using this heatsink would be to get something to cover the bottom of the board as this heatsink only sits on top. If you think there is a risk of shorting the board from the bottom, you might find something to cover it.
Otherwise, great product
There is the 5v option and the 3v option. Preliminary tests indicate to achieve the quietest noise level you'll want to use the 3v option. You can cross-reference the seller images or google the placement per board.
For raspberry pi 3+ and 4+ boards, I use the 3v option when applicable. A word of caution. Only peel off the adhesive tape when you are 100% ready to apply.
It is sticky and difficult to remove. Depending on the application, you might cause damage to the board if you attach it too early and have to change it later. I hope this helps!
Removing this thing was not easy. Not in the slightest. It holds itself in place by using the foam type substance and it leaves the stuff ALL OVER the processor and sdram. I did my best to clean it off, and very carefully. After installing it in another case, the pi booted halfway into my retropie image, then froze.
After removing power and then trying to reboot, I got an sdram error. I borrowed a raspberry pi 4 from a friend, and can confirm the case does work with it.
My only conclusion is that in trying to clean the awful foam adhesive stuff this thing uses that I have accidentally messed up my pi 4.
They really need to find some other way to fasten this thing to the board. Now I don't have a pi 4 during a shortage where these things cost three-four times more than they should
Regret all around.

In open air though, or even just with the case top off, I'm seeing a very substantial temperature drop about 20oC cooler than before with just a passive copper heatsink.
Virtually silent, I can't hear it unless my ear is almost close enough to feel the air that's being sucked into the fan, almost touching the blades.
Took all of 3 minutes to fit to my Pi 3 B+
Very happy with this cooler.
